Observation

Staff member Alyssa reported she did not know the female child had gone to the restroom because she was not asked by the child. This is not the frist incident and has been a known ongoing occurance with how children are supervised when going to the restroom. During an onsite inspection licensor Diaz observed the children do not use the sign outside the door as intended to help notify staff when they are in use.
